What an HSCT for MS Costs Around the World

Two things stand out in the hsct cost by country comparison below. India is the lowest cost, at roughly half the Mexican price and about a fifth of the American one. And the American figure is why almost nobody pays it: in the United States, HSCT for MS is largely confined to clinical trials, and if you do not qualify for a trial, the list price is out of reach for most families.

Why Is HSCT So Much Cheaper in India?

This is the question every patient asks, and it deserves a straight answer, because a low price in medicine should make you cautious. The cost of an HSCT is driven by three things: the chemotherapy drugs, the hospital bed, and the staff. In India, the drugs cost the same as they do anywhere, because they are globally traded, but a hospital bed and a specialist salary cost a fraction of what they do in Boston or Munich. That is the entire explanation.

It is not a cheaper protocol, a shorter stay, or a lower standard of care. It is a lower cost base, in the same way that almost everything in India costs less than it does in the United States. What you should check, in any country, is what the price actually buys. What Does the Price Actually Include?

Two prices that look similar can buy very different things. One centre quotes a figure that covers only the transplant itself; another includes the room, the attendant, the medicines and the follow-up. Here is what is inside our package, and the exact questions to ask any other centre.

Inpatient or Outpatient: The Difference Nobody Explains

This is the single most important practical difference between centres, and it is rarely spelled out on a price list. After the conditioning chemotherapy, your immune system is at its lowest. For roughly two weeks, your white cell count is on the floor and you have very little defence against infection. That window, the aplastic phase, is when HSCT patients get into trouble, and it is the reason bone marrow transplant units exist in the first place: filtered air, controlled access, and nurses who can recognise a fever turning into sepsis at two in the morning.

At HSCT Hospital India you spend that entire window as an inpatient, in a private room with triple HEPA air filtration and 24 hour nursing. You do not leave the hospital and come back. Clinica Ruiz in Mexico runs a largely outpatient programme: patients stay in a dedicated apartment building near the clinic and attend for their treatment. To be fair to them, they have done this a very long time and they do it well, reporting more than 3,500 transplants, over 1,900 of them for MS, and a transplant-related mortality under 0.26%, which is a strong safety record.

It is a different model, and you should choose it with your eyes open. The question to ask yourself is simple: if you spike a fever at three in the morning with no immune system, where would you rather be? Is the Protocol the Same Everywhere?

Broadly, the leading centres, including ours, Clinica Ruiz and the Pirogov Centre, all use a non-myeloablative protocol. Non-myeloablative means the conditioning chemotherapy dose is calibrated to reset your immune system rather than to wipe out your bone marrow. It is the approach with the best safety record in autoimmune disease, and it is the one used in the major trials, including the MIST randomised trial published in JAMA, which found non-myeloablative HSCT superior to continued disease-modifying therapy for relapsing multiple sclerosis.

Where centres differ is in the detail: the exact conditioning drugs, whether they use rituximab or anti-thymocyte globulin, and whether they require follow-up chemotherapy afterwards. Our protocol requires none. The long-term outcome data published in JAMA Neurology and a real-world series of 507 patients both support the durability of this approach. What About the Waiting List?

In most national health systems, HSCT for MS is either unavailable or available only inside a clinical trial with strict entry criteria and a long wait. That is the reason people travel. Our waiting period is measured in weeks, not years. If your MS is actively relapsing, that difference is not administrative. Every relapse you have while waiting can leave damage that no transplant can later undo, which is why we assess disease activity carefully against your EDSS score.

Is HSCT Right for You? A Quick Eligibility Check

HSCT is not the right fit for every patient, and we would never suggest it was. We weigh eligibility with care, and patient safety guides every decision we reach. The key factors we look at include:

  • A confirmed diagnosis
  • Disease that is still active and inflammatory, rather than driven purely by accumulated damage
  • An EDSS score generally at or below 6.5 for MS patients
  • Insufficient response to standard treatments
  • Overall health, including heart, lung, liver and kidney function
  • Age and general fitness

Before anything is decided, every prospective patient at HSCT Hospital India is reviewed by the full multi-disciplinary team. How much does HSCT cost in Russia?

Treatment at the A.A. Maximov department of the Pirogov Centre in Moscow is typically quoted at 40,000 to 45,000 US dollars for a stay of five to six weeks, as an inpatient.

How much does HSCT cost in the USA?

Between 150,000 and 200,000 US dollars for a stay of six to seven weeks. In practice, HSCT for MS in the United States is largely restricted to clinical trials, so most patients who want it cannot access it at any price.

Why is HSCT so much cheaper in India?

Because a hospital bed and a specialist salary cost far less in India than in the United States or Germany. The chemotherapy drugs cost the same everywhere. It is not a cheaper protocol or a shorter stay, it is a lower cost base.

Is HSCT in India safe?

HSCT carries real risk anywhere in the world, and the greatest risk is infection during the two weeks after conditioning when your immune system is at its lowest. What is the difference between inpatient and outpatient HSCT?

The inpatient model means that if you develop a fever in the middle of the night, you are already in a hospital with medical staff around you.

Do I need follow-up chemotherapy after HSCT?
